Job Summary

The Case Manager provides supportive case management services to tenants of a permanent affordable supportive housing program. The role involves working with tenants to build independent living skills, promote housing stability, and ensure timely rent payments and healthy housekeeping.

Compensation and Schedule
  • Pay: $24.04 per hour (~$50,004 annually)
  • FLSA: Non-exempt, eligible for overtime
  • Schedule: Tuesday - Saturday, 10am-6pm (Off Sunday & Monday)
  • Location: Eastchester neighborhood, Bronx, NY
Essential Duties
Tenant Services
  • Screen DHS referrals for eligibility and certification
  • Conduct tenant intake and needs assessments; develop ILPs
  • Provide case management and track progress
  • Coordinate workshops and trainings on independent living skills
  • Engage tenants in supportive programs; refer to community services
  • Advocate for tenants’ entitlements and benefits
  • Link tenants to employment support services
  • Follow up on incidents; submit reports timely
  • Ensure compliance with contracts, laws, and regulations
  • Attend trainings and obtain certifications as required
Tenancy & Apartment Management
  • Ensure rent payments are made timely; develop rent budgets
  • Assist with rent payment agreements and compliance
  • Counsel tenants on housekeeping and neighbor relations
  • Coordinate with property management for essential services and repairs
Minimum Requirements
  • Bachelor’s Degree in Social Work or Counseling preferred; 3+ years experience can substitute
  • Two years of experience in social work, human services, counseling, or case management
  • Excellent verbal and written communication skills; proficiency in English; multilingual skills a plus
  • Computer proficiency in MS Office; experience with CARES and AWARDS preferred
  • Ability to develop and manage program budgets
  • Physical ability to conduct unit visits (walk, climb stairs)
  • Strong reasoning skills for prioritization and decision making
  • Experience with homeless populations, crisis intervention, de-escalation, and behavioral management
  • Ability to handle a demanding caseload, work independently and in teams, and stay organized
