PURPOSE:

Youth Assertive Community Treatment (ACT) services aim to improve functional impairments and severe symptoms caused by mental illness or emotional disturbances in youth. The program also focuses on enhancing family functioning to promote health, stability, and reintegration for youth transitioning from residential or inpatient care. The Youth ACT Team is a multidisciplinary group providing family-driven, youth-guided, developmentally appropriate services addressing needs across family, school, medical, behavioral, psychosocial, and community domains.

POSITION OVERVIEW:

The Youth ACT Team Clinical Support/Case Manager collaborates with a multidisciplinary team to deliver treatment and support to families and children aged 10 to 21 with significant behavioral health needs. The role involves risk assessment, developing person-centered care plans, educating families, and coordinating health and community services, including linking to community resources. The Clinical Support/Case Manager supports youth transitioning home, engaging with them and their families through individualized services focused on clinical treatment, psychoeducation, and skills development. Services are provided in homes and communities, requiring some evening hours and rotating on-call duties.

KEY ESSENTIAL FUNCTIONS:
  • Provide linkages to community resources to support youth living in the community, transitioning from higher levels of care, and achieving personal goals.
  • Ensure services in educational, vocational, and benefits domains are identified and addressed.
  • Engage and assist youth and families in defining goals and action steps.
  • Participate actively as part of a multidisciplinary team, with a minimum of 6 team meetings per month.
  • Assist youth and families in obtaining medical, social, psychosocial, educational, financial, vocational, housing, and other services.
  • Establish collaborative relationships with community providers, Managed Care Plans, schools, and medical providers.
  • Complete documentation and report data in compliance with standards.
  • Foster relationships with community providers to facilitate smooth transitions and information sharing.
  • Navigate appointments, including travel training and re-engagement, making appropriate referrals.
  • Attend interdisciplinary team meetings and supervisory sessions.
  • Monitor, evaluate, and record progress toward care plan goals.
  • Attend weekly Youth ACT team meetings and mandatory trainings.
  • Work flexibly to meet participant needs.
  • Perform other related duties as assigned.

EDUCATIONAL/TRAINING REQUIRED:
  • Bachelor's degree required; master's preferred.
EXPERIENCE REQUIRED/LANGUAGE PREFERENCE:
  • At least one year of experience working with families and children with serious emotional disturbances or severe mental health issues.
COMPUTER SKILLS REQUIRED:
  • Proficiency with Microsoft Office; ability to learn electronic health records and other software; strong documentation skills.
WORK ENVIRONMENT/PHYSICAL EFFORT:
  • Frequent travel within Bronx or Queens; occasional travel across NYC.
