Summary / Objective
East Side House (ESH) is a community resource serving the Bronx and Northern Manhattan. ESH’s Social Services programs align with our mission by providing high-quality programs and services through community-based programs and school partnerships.
The Case Manager serves as a primary point of contact for individuals and families participating in the Housing Retention & Stabilization Services program. The Case Manager works collaboratively with participants to identify immediate and long-term needs, develop individualized service plans, and connect households to financial assistance, employment, education, public benefits, healthcare, and community-based resources.
Case Managers work closely with Lead Social Workers and Social Workers when participants present complex psychosocial, behavioral health, crisis, safety, or family needs requiring specialized or higher-level social work assessment and intervention.
