Job summary

Signature Behavioral Health in Brick/Morris Plains, NJ is seeking a dedicated Case Manager to support adults with serious mental illness. The role involves establishing therapeutic relationships, managing up to 20 clients, and coordinating recovery plans within a fast-paced setting.

You will lead psychoeducation groups, document progress, and advocate for clients to access needed entitlements, while partnering with social workers and clinicians across the care team.

Qualifications

• Bachelor’s or Master’s degree in social work, psychology, counseling or related field.
• Minimum of two years of relevant experience in partial care or supervised residential setting, working with seriously and persistently mentally ill.

Responsibilities

• Establish and maintain an ongoing therapeutic relationship and case manage up to 20 mixed risk level persons served.
• Conduct psychoeducation groups and provide individualized case management sessions.
• Maintain up-to-date records, including weekly progress notes and develop individualized recovery plans.

• Assigning, monitoring, and organizing cases, directly involved in clients' lives, providing resources and assistance.

Qualifications: Bachelor’s degree or Master's degree in social work, psychology, counseling or a related field, and a minimum of two years of relevant experience in partial care or a supervised residential setting, working with the seriously and persistently mentally ill.

The case manager will be responsible for assigning, monitoring, and organizing cases for social work. They will be directly involved in the lives of their clients, and provide resources and assistance. Planning health services, working with social workers and doctors, and scheduling treatment or recovery is all in a day's work. The case manager must be highly organized, empathetic, compassionate, nonjudgmental, and eager to help vulnerable adults.

Case Manager Duties and Responsibilities

• Accessing client needs, and developing strategies to help the client
• Providing support and resources for clients; good working relationship with support networks, government resources, and community resources
• Researching and referring client resources
• Respond to crisis situations that affect vulnerable or mentally unstable adults
• Evaluating and studying programs, services, and resources for quality and client needs
• Determining the correct course of action when helping clients
• Caring for the well-being of clients and helping a wide range of clients, including people with serious illnesses, addictions, disabilities or who have been neglected
• Advocating and raising awareness on behalf of clients and the needs of the community and local services
• Assisting clients by providing and facilitating various psycho-education groups that focus on clients mental health needs

Case Manager Requirements and Qualifications

• Possesses superb written and spoken communication skills

• Bachelor's degree in social work, psychology, sociology, healthcare or medical and related fields; Master's degree in social work preferred
• Proficient computer skills, including Microsoft Office Suite (Word, PowerPoint, and Excel)
