Job summary

Indiana Professional Management Group seeks a Case Manager to work a full-time hybrid schedule, combining remote work with travel to community meetings within Henry County, Indiana. Applicants must reside in Henry County and be able to travel to surrounding counties as needed.

You will educate individuals about Medicaid waivers, develop tailored support plans, and coordinate access to services, while leading regular team meetings with providers to ensure client goals are met.

Qualifications

• Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field.
• Bachelor's degree in an unrelated field and a minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population.
• A valid driver's license, car insurance, and reliable transportation.
• The ability to pass thorough background screens.
• Designated home office setting with computer, internet service, and smart phone.

Let’s talk about the Case Manager position!

This full-time, hybrid position combines remote work from your home office with travel of up to approximately one hour for community-based meetings several times a week. This position serves Henry County, and to accommodate required in-person meetings, applicants must reside within Henry County, Indiana. Candidates must be able to travel throughout the county and surrounding counties as needed to attend community-based meetings.

What You’ll Do:

Who You’ll Work With:

• Case managers work with Individuals on four Medicaid Waivers; Family Supports Waiver (FSW), Community Integrated Habilitation Waiver (CIH), Health & Wellness Waiver (H&W), and Traumatic Brain Injury Waiver (TBI)

• A Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field, or
• A Bachelor's degree in an unrelated field and a minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population.
