Responsibilities include life-skills coaching, placement coordination, transportation assistance, and diligent reporting to supervisors and the Drug Court Team.

Description

The Case Manager coordinates placements for participants after completion of the in-jail portion of the program and monitors their progress at that placement. Performs substance (drug) screenings for participants on a daily basis. Assessments, advocacy, monitoring, collaborating care coordination, support, and documentation are essential and routine. Additional responsibilities include but are not limited to:

  • Process participants substance (drug) screens in the onsite lab on a daily basis as participants respond to their random screening requirement;
  • Teach in-jail classes and evening classes with participants who have been released;
  • Assist participants with understanding and performing basic life skills in order to re-enter society substance-free;
  • Perform placement coordination and monitoring;
  • Provide transportation for participants to placement;
  • Check-in and maintain reports on participants;
  • Report to the supervisor and Drug Court Team regarding individual participant progression;
  • Attend workshops and training sessions as appropriate; and
  • Perform other assigned duties.

MINIMUM REQUIREMENTS TO PERFORM WORK

  • Degree favorable but not required;

  • Valid driver’s license.

PHYSICAL DEMANDS

While performing the duties of this position, the employee is frequently required to sit, reach and manipulate objects, tools and/or controls. The position requires mobility. Duties may involve moving materials weighing up to 10 pounds on a regular basis. Manual dexterity and coordination are required over 50% of the work period while operating equipment, such as computer keyboard, mouse, calculator and similar machines. The position requires hearing, mental acuity, speaking, and visual acuity.

WORK ENVIRONMENT

Most work is performed in a well-lighted, modern office setting with centrally controlled heating and air conditioning; and frequent work is performed in the County jail and courtrooms.
