Case Manager

JOB SUMMARY:

This position provides administrative, clerical, and case management support to the Program Coordinator within Treatment Services. The incumbent prepares, updates, and maintains participant files; manages program databases; coordinates participant services; schedules community service and drug screening; maintains confidentiality of participant and office information; and provides accurate reporting to support program operations and grant requirements. [If assigned to Drug Court, all case managers will also work closely with senior case manager regarding daily tasks as it relates to Drug Court.]

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
  • Facilitate participant job search requirements.
  • Track compliance with program sanctions and community service requirements.
  • Schedule participant drug screening and ensure accurate tracking of results and compliance.
  • Facilitate program orientation for new program participants.
  • Serve as initial point of contact for participant calls and office visits, including medication approvals, address changes, and leave requests.
  • Notify participants of program sanctions and tracks incentives and sanctions.
  • Prepare, update, and maintain participant files in accordance with agency policy.
  • Maintain accurate participant demographic and program status data for grant reporting.
  • Maintain program database and ensure timely, accurate updates.
  • Track participant fee collection, maintain documentation, and prepare program deposits per agency policy.
  • Obtain and maintain accurate information from treatment providers.
  • Maintain confidentiality of all participants and office information as required by federal law.
  • Attend status review meetings and court sessions as scheduled.
  • Prepare court spreadsheets, certificates, cards, incentives, and related materials.
  • Prepare program graduation materials, including certificates, programs, and refreshments.
  • Provide accurate reports to the Program Coordinator to support grant objectives.
  • Develop individualized case plans with each participant.
  • Facilitate regular meetings with program participants to review case plan goals.
  • Provide community resources to participants when necessary.
  • Maintain home visit records for Community Policing teams and enter information into the agency database.
  • Assist with drug screen collection as needed.
  • Develop and maintain effective working relationships with internal departments, external agencies, and the public.
  • Operate a personal computer and various software applications, including database systems and office productivity tools.
  • Maintain regular and predictable attendance.
  • Perform other related duties as required.
Minimum Qualifications:
Education and Experience:
  • · Bachelor’s degree in Criminal Justice, Psychology, Social Work, Business Administration, or a related field preferred. Two years of case management experience preferred with knowledge of criminal justice, substance use disorder, mental health, and/or child abuse/neglect cases or an equivalent combination of education, training, and experience.
  • · Background in or understanding of addictions and mental health preferred.
  • · Must be familiar with HIPAA and Federal Confidentiality guidelines.
Licenses and Certifications
  • · Must maintain all professionally related certifications.
Knowledge, Skills and Abilities:
  • County organizational structure and Treatment Services operations.
  • Must be detail-oriented and able to work in a fast-paced environment with limited supervision.
  • Criminal justice, substance use disorder, mental health, and case management processes.
  • Bilingual proficiency in Spanish and English, preferred for DUI Court.
  • Strong organizational skills and attention to detail.
  • Proficiency with personal computers, databases, and related software applications.
  • Strong oral and written communication skills.
  • Ability to develop and maintain positive customer relations.
  • Maintain confidentiality and handle sensitive information.
  • Develop and maintain effective working relationships.
  • Provide accurate reporting and documentation.
  • Work independently and collaboratively in a team environment.
Physical Ability

Functions frequently require moving about the office and transporting items up to 10 lbs.

Functions occasionally require bending, twisting, reaching overhead, pushing/pulling, and repetitive use of hands/arms/legs. Incumbents may be required to be on-call and work irregular hours.

Sensory Requirements

Essential functions require effective verbal and written communication and hand-eye coordination for equipment use.
