Abraxas Youth and Family Services in Marienville, PA is seeking a Case Manager, Mental Health to support admissions, assessments, and coordinated care for youth and families. This full-time role emphasizes compassionate, team-oriented service and aligns with a mission-driven organization.

Qualifications

• Compassionate, dependable, patient, team-oriented and service-focused candidates.
• Bachelor’s degree from an accredited program is required; degree in human services preferred.
• Criminal clearances and valid driver’s license required.

Responsibilities

• Completes admission and assessment documentation.
• Verifies eligibility for Managed Care and Medical Assistance benefits and coordinates with appropriate agencies.
• Has oversight of referrals and family-related invitations and mailings.
• Participates in case consultations including treatment reviews and administrative reviews.
• Maintains regular contact with parents, probation officers, case workers, and treatment team.
• Facilitates group counseling sessions per individualized treatment plan.
• Supports development and implementation of client’s treatment plans.

What You’ll Do

• Completes admission and assessment documentation.
• Verifies eligibility for Manage Care and Medical Assistance benefits and works closely withappropriate agencies.
• Has oversight of invitations and mailings to referral sources and families.
• Participates in and documents case consultations including treatment reviews, red flag meetings, and administrative reviews.
• Assistsin documenting court reports, psychiatric evaluations, and discharge summaries.
• Maintains regular contact and communicates effectively with parents, juvenile probation officers, case workers, advocates, and members of the client’s treatment team.
• Facilitates group counseling sessionsin accordance withtheclientsindividualized treatment plan.
• Interacts meaningfully with clients;observesbehavior and intervenes appropriately.
• Assistsandparticipateswith the development and implementation ofclient’streatment plans.
• Supports the Abraxas Philosophy, Mission Statement, and promotes the Seven Key Principles ofcare.
• Demonstratesappropriate useof Safe Crisis Management (SCM) and provides effective supervision to staffregardingthe use of SCM.

Qualifications

The following educational requirements are acceptable for this classification including:

• Bachelor’s degree from an accredited programrequired; degree in an area of human services preferred.

Other Qualifications

• At least twenty-one (21) years of age.
• Criminal clearances (Act 33 and 34, specific State & child clearances and, if necessary, FBI).
• Non-communicable diseases physical exam.
• Valid driver's license from employee’s state of residence.
• Ability to work overtime asrequired.
• Ability to work up to sixteen (16) hours within a rolling24 hourperiod.
• Ability to handle physical and mental stress associated with working extended hours.
• Ability to work with computers and the necessary software typically used by the department.
