Job summary

Well Care Services is a Behavioral Health Clinic seeking a full-time Mental Health Case Manager to work with clients in the field. You will assess clients, develop individualized service plans, and coordinate access to services and resources.

Responsibilities include linking clients to community services, monitoring progress toward goals, and collaborating with professionals. A drug and background check is required, and driving is necessary for fieldwork.

Qualifications

• Minimum of two years of experience working with adults with serious mental illness; or an equivalent combination of training and experience.
• Knowledge of serious mental illness, substance abuse, psychotropic drugs, entitlement programs and recovery oriented services.
• Interviewing, service planning, consultation, public relations, and experience with electronic medical record.
• Computer skills and the ability to type.
• Good driving.
• Must not reflect a total accumulation of six or more negative points within the past three years or a major violation within the past four years.
