Job summary

Lake Shore of Lake Ronkonkoma invites you to join as a Case Manager, where you will build meaningful relationships with residents and families while coordinating care and solving problems as a trusted leadership team member.

You will advocate for older adults and help them maintain dignity, independence, and quality of life, collaborating with nursing, recreation, dietary, housekeeping, and other departments to ensure exceptional care.

Qualifications

• Compassionate, resident-focused professional.
• Excellent communication and problem-solving skills.
• Strong organizational and documentation abilities.
• Experience in Assisted Living, Long-Term Care, Social Services, Case Management, or Healthcare preferred.
• Ability to build trusting relationships with residents, families, and team members.

Responsibilities

• Build strong relationships with residents and their families.

• Conduct resident assessments and routine follow-up visits.
