Job summary

Qualifications

  • Prior experience in case management, care coordination, social services, assisted living, long-term care, home care, or a related healthcare setting strongly preferred.
  • Experience working with older adults and/or individuals requiring supportive services preferred.
  • Strong organizational and case-management skills with the ability to manage multiple residents and competing priorities.
  • Excellent written and verbal communication skills.
  • Strong documentation and follow-up skills.
  • Ability to communicate professionally and compassionately with residents, families, healthcare professionals, and colleagues.
  • Comfortable working in a fast-paced, highly regulated environment.
  • Proficiency with electronic documentation systems and Microsoft Office preferred.
  • Bilingual skills are a plus.

Responsibilities

  • Manage an assigned caseload of residents and help coordinate their ongoing care and service needs.
  • Serve as a liaison between residents, families, physicians, healthcare providers, managed care organizations, and community resources.
  • Monitor resident needs and promptly communicate changes or concerns to the appropriate clinical and leadership team members.
  • Participate in resident assessments, care planning, case conferences, and interdisciplinary meetings as required.
  • Maintain accurate, complete, and timely resident documentation in accordance with facility policies and regulatory requirements.
  • Assist with obtaining and maintaining required medical, clinical, and supporting documentation.
  • Coordinate appointments, services, referrals, and follow-up care as appropriate.
  • Communicate with residents and families regarding services, concerns, and changes in resident needs.
  • Assist with resolving resident and family concerns and ensure appropriate follow-up.
  • Support admission, transition, hospitalization, and discharge processes as applicable.
  • Work collaboratively with Nursing, Home Care, Admissions, Medicaid/Benefits, and other departments to promote continuity of care.
  • Maintain resident confidentiality and comply with HIPAA and all applicable policies and regulations.
  • Identify concerns that may affect resident health, safety, or continued placement and identify them appropriately.
  • Participate in audits, quality initiatives, and regulatory compliance activities.
  • Perform other duties as assigned to support residents and community operations.

About the Role

Amber Court of Pelham Gardens is seeking a compassionate, organized, and dependable Case Manager to join our team. This position plays an important role in coordinating services and supporting the ongoing needs of our residents while serving as a key point of communication between residents, families, healthcare providers, community partners, and our interdisciplinary team.

The ideal candidate is someone who genuinely enjoys working with older adults, is comfortable managing multiple priorities, and understands the importance of timely follow-up and accurate documentation in a regulated healthcare environment.
