Job Title: Case Manager I

Reports to: Program Manager

Full or Part-time: Full-Time

Team: Men's Residential Center

Position Summary:

The Case Manager provides a continuum of care through assessment, resource identification, and goal setting to help guests achieve stability.

Key Duties and Responsibilities:

• Assess and identify guests' resources, talents, skills, and interests.
• Develop and monitor care plans with goals and methods for success.
• Advise and advocate for guests, providing referrals and crisis intervention as needed.
• Monitor guest progress and ensure program compliance while maintaining confidentiality.
• Facilitate small groups and assist guests in establishing a foundation in Christ.
• Coordinate services with other staff and agencies, and maintain accurate records.
• Participate in team activities, including prayer, study, and community involvement.

Reporting Relationships: The Case Manager may supervise program assistants, interns, and volunteers.

Required Competencies:

• Adaptability, organizational agility, guest focus, collaboration, empathy, patience, prioritization, conflict resolution.

Education & Experience:

• High School Diploma or HSE.
• 1-2 years in social service/ministry, including counseling, mentoring, discipling.
• Experience working with individuals facing substance abuse, mental health issues, trauma, domestic violence, or homelessness.

Technical Skills:

• Proficiency in Microsoft Office, Resident Database, and basic Paycom.

Knowledge Areas:

• Community resources for housing and employment.
• Best practices in case management and faith-based service.

Physical Requirements:

• Walking, typing, lifting up to 20 pounds, driving.

Other Requirements:

• Full-time, with evening/weekend hours.
• Valid driver’s license, insurable driving record, background check, drug-free policy adherence.
