Job Title: Case Manager II

Position Description

Wheeler Mission Ministries is a privately funded 501(c)(3) non-profit, evangelical Christian organization dedicated to sharing the Gospel and assisting the homeless and impoverished. All employees are expected to have a personal relationship with Jesus Christ and subscribe to our Mission, Vision, Statement of Faith, and Core Values. Position Summary:

Responsible for providing a continuum of care through case management, including assessment, resource identification, and goal setting. Includes mentoring, discipleship, counseling, and group management.

Key Duties and Responsibilities:

  • Case Management for Guests: Conduct assessments, develop care plans, monitor progress, advise on success strategies, and facilitate resource referrals.
  • Program Oversight: Manage program elements, monitor guest progress, ensure compliance, facilitate groups, and help guests find a Foundation in Christ.
  • Coordination of Services: Collaborate with staff and external agencies, advocate for guests, and coordinate services.
  • Administrative Tasks: Maintain guest records, ensure confidentiality, participate in case conferences, and practice self-care with prayer and study.

Additional responsibilities include leading program elements, resource development, attending court hearings, gathering program data, and mentoring junior staff.

Reporting Relationships: May supervise program assistants, interns, and volunteers.

Required Competencies: Adaptability, organizational agility, listening, guest focus, collaboration, patience, interpersonal skills, prioritization, conflict resolution, compassion, networking.

Qualifications:

  • Minimum 2 years in social service/ministry, including counseling, mentoring, discipling.
  • At least 1 year working with individuals facing substance abuse, mental health issues, trauma, domestic violence, or homelessness.
  • Bachelor’s degree preferred for Level II.

Knowledge and Skills: Knowledge of community resources, case management best practices, trauma-informed care, mental health support, proficiency in Microsoft Office, database use, and basic payroll software.

Mission Alignment: Belief in the inerrancy of the Bible, alignment with Wheeler Mission values, adherence to policies.

Physical Requirements: Ability to walk, operate office equipment, stand for long periods, lift up to 20 pounds, and drive.

Other Requirements: Full-time hours, valid driver’s license, background check, drug testing, adherence to policies.
