Case managers are an important part of the mental health field. They act as a safety net for those experiencing hardship and challenges in their life, providing comfort, suggestions, and referrals to outside agencies that will offer the best support or outcome for each situation. The goal of case management is to ensure a person is receiving the support they need while working to increase the individual's ability for self‑support and self‑advocacy.

Responsibilities – Case Manager
  • Coordinate the delivery of each person’s treatment plan
  • Work as a team with the client, their family and other providers to ensure the correct care plan is in place
  • Link client to support systems that will promote growth
  • Coordinating and monitoring the client’s ability to navigate and use services
  • Evaluating progress quarterly while adjusting care as needed to improve care
  • Provide crisis intervention by assessing the situation
  • Be flexible with scheduling and perform and prioritize multiple functions and tasks
  • Be able to work independently, follow verbal and written instructions and meet deadlines
  • Comfortable navigating electronic documents and electronic systems required to perform job duties
  • Maintain strong boundaries and understand cultural diversity
  • Have a strong overview of mental illnesses and their effects on the brain
  • Maintain confidential information within HIPAA guidelines and organisational policies
  • Adhere to the PRA Ethics and Professional Conduct
  • Value working with underserved and marginalised populations and appreciate diversity
  • Adhere to mandatory abuse‑reporting laws
  • Have a mature attitude and sensitivity to populations dealing with mental health challenges
  • Submit records of weekly interventions, initial recovery plans and quarterly recovery plan reviews in a timely manner
  • Attend monthly individual and group supervision with a clinician
  • Attend periodic team, department and agency‑wide staff meetings
  • Strong communication skills and basic keyboarding skills
Requirements
  • Bachelor’s degree in health and human services is required
  • Able to work in the United States
  • Successful completion of drug screening and DHW background check
  • Current Idaho driver’s licence and the ability to meet insurance requirements for a personal vehicle
