Job Description

• Reports to and works closely with the Coordinator of Case-Management and Student Development and other administrative staff.
• Works closely with the Director, Assistant Director, and Coordinators to ensure quality care is being provided to clients.
• Participates in the on-call schedule and manages crisis situations.
• Coordinates and facilitates patient care through assessment, evaluation, planning, and implementation.
• Participates in Counseling Center outreach events.
• Tracks events and goal progress for the Counseling Center.
• Schedules meetings and sends out Teams links weekly.
• Works closely with the Mental Health Specialists, carrying caseload for medical management and higher acuity clients for more hands‑on services.
• Conducts walk‑in clinic intakes assessment.
• Develops, monitors, and evaluates treatment plans and progress.
• Communicates client needs to a variety of care team members and follows up accordingly, providing appropriate referrals.
• Links and refers students to community resources.
• Maintains client records by reviewing case notes and monitoring files.
• Advocates for client’s success.
• Schedules/reschedules psychiatric appointments.
• Handles psychiatric medication refills.
• Prepares weekly psychiatric provider reports and maintains weekly communication with medical staff.
• Monitors psychiatric appointments and communicates any concerns with Mental Health Specialists and medical staff.
• Documents and monitors student attendance; outreaches to clients who miss appointments to ensure continuity of care.
• Manages discharge plans upon completion of treatment.
• Assists in development and execution of workshops and support groups.
