Job summary

The Retreat is seeking a full-time Case Manager located in Hauppauge, New York. This role focuses on supporting low-income fathers involved in the Long Island Fatherhood Initiative (L.I.F.I) program by providing outreach, intake, and personal assessments.

The Case Manager will work to facilitate program retention and access to services that enhance economic stability and promote responsible parenting. The ideal candidate should have experience in case management and strong communication skills.

Job description

Position: Case Manager | Full-time | Benefits Eligible | 35 hours per week

FLSA status: Non-Exempt

Hours: M-F, Days

Department/Location: Fatherhood/Hauppauge

PURPOSE

The Long Island Fatherhood Initiative (L.I.F.I) program will address the needs of fathers 18 years & older (most low‑income, many with history and involvement in the criminal justice system) in both Nassau & Suffolk Counties. The case manager's primary goal is to work with fathers by providing regular contact to facilitate program retention, completion, and access to needed support services with greater focus on economic stability and job‑focused services with a clear and unwavering commitment to promoting responsible parenting and healthy marriage/relationship.

Specific Responsibilities

• To provide systematic, coordinated outreach, intake, and comprehensive personal assessment services for all eligible participants referred by community‑based agencies or identified through extensive outreach efforts.
