Job summary

True North Recovery Inc. is seeking a dedicated Case Manager to provide comprehensive case management and advocacy for participants in our Alaska program.

You will coordinate resources, document interactions, and support participants toward stable housing and employment while ensuring safety and confidentiality. Responsibilities include collaborating with agencies, performing crisis planning, and supporting program completion with personalized aftercare plans.

Qualifications

  • Associate's degree in human services or related field or equivalent combination of education and experience.
  • One year of continuous sobriety if in recovery and active participation in a recovery program.
  • Must obtain CDC II within 3 years of hire.
  • Valid Alaska driver’s license and insurable under company automotive policy.
  • Continued employment contingent on satisfactory state and federal background check.

Responsibilities

  • Maintains relationships with partnering agencies and referral sources.
  • Documents billable interactions into AKAIMS using DAP note format.
  • Coordinates with referral sources, family members, community agencies and other providers to tailor treatment options.
  • Provides motivational interviewing, social and emotional support to participants.
  • Creates crisis stabilization plans and coordinates peer supports for immediate goals.
  • Assists participants in identifying and linking to housing, employment, medical and social services.
  • Helps participants develop personalized plans for housing, employment, support and treatment after completion.
  • Assists with exploring available grants for housing, transportation, education, and other needs.
  • Attends trainings and staff meetings; participates in ongoing service delivery improvements.
  • Transports participants for activities and outings as needed.
  • Maintains respectful relationships with participants, families and peers.
  • Performs other duties as assigned.

Job Description:

Responsible for providing case management services to participants within the program and will act as an advocate for the participant providing resources, case management, transportation as well as working with participants on pre- and post-treatment planning.


ROLES AND RESPONSIBILITIES:


  • Maintains a relationship with all partnering agencies and referral sources.

  • Efficiently document all billable interactions into AKAIMS, utilizing DAP note formatting.

  • Coordinates with referral sources, involved family members, community agencies and other treatment providers to provide the best treatment and case management options for the individual participant’s needs.

  • Provides motivational interviewing, social, and emotional support to participants entering and engaging in program.

  • Create crisis stabilization plans as needed for participants exploring treatment and coordinate assistance from peer supports to accomplish immediate/crisis goals.

  • Assists participants in identifying case management services; as needed, in accordance to best practices to provide linkage and advocacy that participants need. To include coordinating referrals that pertain to employment, mental health, housing, medical with local community clinics, social service agencies, continuing care and provide follow-up on all referrals made.

  • Works with participants to develop and update personalized plans prior to program completion to ensure participants leave the program with a clear plan for continued housing, employment, support, and treatment needs.

  • Assist participants in exploring available grants to aid them in getting funding for housing, transportation, education, and other needed items.

  • Attends all relevant trainings and direct support meetings.

  • Participate in training in areas related to improvement of service delivery.

  • Attends staff meetings and works collaboratively with other staff in case staffing/review.

  • Transport participants on assigned activities, case management services and outings as needed.

  • Maintains a respectful relationship with participants, former participants, family members and peers.

  • Performs other duties as needed and/or assigned.


QUALIFICATIONS AND EDUCATION REQUIREMENTS:


  • Associate's degree in human services or related field. An equivalent combination of education and experience may be accepted.

  • If in recovery, one year of continuous sobriety prior to hire and active and continuing participation in a recovery support program, such as a 12-step program.

  • Must obtain CDC II within 3 years of hire.

  • Valid Alaska driver’s license and insurable under TRUE NORTH RECOVERY INC’s automotive insurance, which requires a driver to be at least 21 years of age.

  • Continued employment is contingent upon receipt of a satisfactory state and federal background check.

PHYSICAL REQUIREMENTS

This role is primarily performed in an office building with standard office environment and controlled temperatures. When working outdoors or at a job site the employee may be exposed to construction equipment or other moving objects/equipment, reference job site specifics. The employee is occasionally required to lift and/or move up to 50 lbs. Employee is frequently required to sit, stand, walk, talk/hear, see, read, and personally interact with others.
