Job summary

True North Recovery seeks a dedicated professional to provide case management and coordination for adults returning from incarceration. You will maintain relationships with partners, document interactions in AKAIMS, and coordinate referrals for housing, employment, and health services.

You will deliver motivational interviewing and support to participants, while navigating multiple grants and program options to ensure successful reentry.

Qualifications

  • Associate degree or equivalent in human services or related field.
  • If in recovery, one year of sobriety and ongoing recovery support involvement.
  • Must obtain CDC II or CDC I & NACA I within 3 years of hire.
  • Valid Alaska driver’s license; insurable under company policy.

Responsibilities

  • Maintain relationships with partnering agencies and referral sources.
  • Document all billable interactions into AKAIMS using DAP format.
  • Coordinate with referrals, families, and providers for suitable treatment and case management.
  • Manage a caseload of adults returning from incarceration and reentering the community.

Job description

Mission Statement

Roles and Responsibilities
  • Maintains a relationship with all partnering agencies and referral sources.
  • Efficiently document all billable interactions into AKAIMS, utilizing DAP note formatting.
  • Coordinates with referral sources, involved family members, community agencies and other treatment providers to provide the best treatment and case management options for the individual participant’s needs.
  • Services a caseload of active participants, all of which will be adults returning from incarceration and entering into the community.
  • Provides motivational interviewing, social, and emotional support to participants entering and engaging in the program.
  • Ability to navigate multiple grants and organization programs.
  • Create crisis stabilization plans as needed for participants exploring treatment and coordinate assistance from peer supports to accomplish immediate/crisis goals.
  • Assists participants in identifying case management services; as needed, in accordance to best practices to provide linkage and advocacy that participants need. To include coordinating referrals that pertain to employment, mental health, housing, medical with local community clinics, social service agencies, continuing care and provide follow-up on all referrals made.
  • Works with participants to develop and update personalized plans prior to program completion to ensure participants leave the program with a clear plan for continued housing, employment, support, and treatment needs.
  • Able to pass a background to enter into correctional facilities and participate in InReach.
  • Assist participants in exploring available grants to aid them in getting funding for housing, transportation, education, and other needed items.
  • Attends all relevant training.
  • Attends Fairbanks reentry coalition meetings and case management meetings. Ability to provide client updates in meetings.
  • Attends staff meetings and works collaboratively with other staff in case staffing/review.
  • Transport participants on assigned activities, case management services and outings as needed.
  • Maintains a respectful relationship with participants, former participants, family members and peers.
  • Performs other duties as needed and/or assigned.
Qualifications And Education Requirements
  • An associate's degree in human services or related field is preferred. An equivalent combination of education and experience may be accepted.
  • If in recovery, one year of continuous sobriety prior to hire and active and continuing participation in a recovery support program, such as a 12‑step program.
  • Must obtain CDC II OR CDC I & NACA I within 3 years of hire.
  • Valid Alaska driver’s license and insurable under TRUE NORTH RECOVERY INC’s automotive insurance, which requires a driver to be at least 21 years of age.
  • Continued employment is contingent upon receipt of a satisfactory state and federal background check.

Physical Requirements

This role is primarily performed in an office building with standard office environment and controlled temperatures. When working outdoors or at a job site the employee may be exposed to construction equipment or other moving objects/equipment; reference job site specifics. The employee is occasionally required to lift and/or move up to 50 lbs. Employee is frequently required to sit, stand, walk, talk/hear, see, read, and personally interact with others.
