About the Role:
The Case Manager plays a pivotal role in coordinating comprehensive care and support services for individuals and families within the health care and social assistance sector. This position is responsible for assessing client needs, developing personalized care plans, and facilitating access to appropriate resources to promote well-being and recovery. The Case Manager collaborates closely with multidisciplinary teams, including mental health professionals, social workers, and community organizations, to ensure seamless service delivery and continuity of care. A key outcome of this role is to empower clients through advocacy, education, and crisis intervention, ultimately improving their quality of life and social functioning. The Case Manager also monitors progress, adjusts plans as necessary, and documents all interactions to maintain compliance with regulatory standards and organizational policies.
