Job summary

Idaho Youth Ranch Inc. is seeking a Case Manager for Anchor House to provide case management for youth and families receiving IYR services. You will coordinate services and support the comprehensive treatment plans in collaboration with the Therapist, Program Manager, client family, and referral sources.

The role emphasizes working with youth and families, completing annual CANS certification, and ensuring coordinated care across providers in Idaho.

Qualifications

• Bachelor's Degree in social work, psychology, counseling, or closely related field with significant coursework in psychology, child development and related studies
• 1 year experience in case management, preferably serving Medicaid clients
• Ability to complete annual certification of CANS
• Passion for working with youth and families

Responsibilities

• Provide case management to clients with behavioral health needs who require coordinated services
• Assess, plan, link, coordinate, and monitor options and services addressing client needs
• Help clients gain access to services and providers and support goal-directed treatment plans
• Work collaboratively with therapists, program managers, clients, families, and referral sources to meet comprehensive needs

This is a collaborative position meaning you will work with the Therapist, Program Manager, client family and referral sources to meet the comprehensive needs of the client.

Must Haves:

• Bachelor’s Degree in social work, psychology, counseling, or a closely related field as identified by inclusion of significant coursework in psychology, child development and related studies
• 1 year experience in case management, preferably serving Medicaid clients
• Ability to complete annual certification of CANS
• Passion for working with youth and families

Our Case Managers:

• Provide case management to clients with a behavioral health diagnosis who are unable to navigate or coordinate the service system independently
• Assess, plan, link, coordinate, and monitor options and services that address a client’s needs
• Help clients learn gain and maintain access to services and providers
• Provide skill building services specific to goals identified in the individualized treatment plan to the point where the client may be safely, efficiently and effectively treated in the least restrictive service level
