Job summary

Idaho Youth Ranch is seeking a Case Manager for Anchor House to support youth and families by coordinating services and delivering individualized case management. The role emphasizes collaboration with therapists, program managers, families, and referral sources to meet clients’ comprehensive needs.

Minimum qualifications include a Bachelor’s in social work or related field, 1 year of case management experience, and commitment to working with Medicaid populations.

Qualifications

• Bachelor’s degree in social work, psychology, counseling, or closely related field as identified by coursework
• Minimum 1 year of case management experience, preferably with Medicaid clients
• Ability to complete annual CANS certification
• Demonstrated passion for working with youth and families

Responsibilities

• Provide case management to youth and families receiving services within Idaho Youth Ranch
• Assess, plan, link, coordinate, and monitor options and services addressing client needs
• Help clients access services and providers and achieve goals in their treatment plans
• Deliver skill-building services aligned with individualized treatment plans to support least restrictive services

How will you impact our mission?

Every position at Idaho Youth Ranch contributes to the success of our mission. As a Case Manager for Anchor House, you can proudly tell your family and friends that you help nurture hope, healing, and resilience in Idaho’s youth by providing case management functions and duties for youth and families receiving IYR services. This is a collaborative position meaning you will work with the Therapist, Program Manager, client family and referral sources to meet the comprehensive needs of the client.

Must Haves

• Bachelor’s Degree in social work, psychology, counseling, or a closely related field as identified by inclusion of significant coursework in psychology, child development and related studies
• 1 year experience in case management, preferably serving Medicaid clients
• Ability to complete annual certification of CANS
• Passion for working with youth and families

Our Case Managers

• Provide case management to clients with a behavioral health diagnosis who are unable to navigate or coordinate the service system independently
• Assess, plan, link, coordinate, and monitor options and services that address a client’s needs
• Help clients learn gain and maintain access to services and providers
• Provide skill building services specific to goals identified in the individualized treatment plan to the point where the client may be safely, efficiently and effectively treated in the least restrictive service level
