This is a full-time, on-site Case Manager role based in Clearwater, FL. The Case Manager will provide direct support to individuals and families facing food insecurity, homelessness, domestic violence, or other basic needs challenges, using a trauma-informed and client-centered approach. Day-to-day responsibilities include conducting intake assessments, developing individualized service plans, coordinating referrals to internal programs and community resources, and monitoring client progress toward established goals. The role involves maintaining accurate and timely documentation, advocating on behalf of clients with partner agencies, and collaborating with team members to ensure continuity of care. The Case Manager will also participate in case conferences, contribute to program improvement efforts, and uphold agency policies, confidentiality standards, and ethical practices.
