Role Description

This is a full-time, on-site Case Manager role based in Clearwater, FL. The Case Manager will provide direct support to individuals and families facing food insecurity, homelessness, domestic violence, or other basic needs challenges, using a trauma-informed and client-centered approach. Day-to-day responsibilities include conducting intake assessments, developing individualized service plans, coordinating referrals to internal programs and community resources, and monitoring client progress toward established goals. The role involves maintaining accurate and timely documentation, advocating on behalf of clients with partner agencies, and collaborating with team members to ensure continuity of care. The Case Manager will also participate in case conferences, contribute to program improvement efforts, and uphold agency policies, confidentiality standards, and ethical practices.

Qualifications
  • Experience in case management, social work, counseling, or a related human services field, preferably working with individuals facing homelessness, hunger, or domestic violence.
  • Strong skills in client assessment, service planning, resource navigation, and crisis intervention, with the ability to work effectively with individuals from diverse backgrounds.
  • Proficiency in maintaining accurate case notes, documentation, and reports, and in using basic computer applications and case management systems.
  • Effective communication, collaboration, and conflict-resolution skills, with the ability to build trust and maintain professional boundaries.
  • Knowledge of local social service networks, community resources, and eligibility processes for assistance programs is highly beneficial.
  • Ability to work on-site in Clearwater, FL, manage multiple cases, prioritize tasks, and adapt to changing client and program needs.
  • High school diploma or equivalent required; an associate or bachelor’s degree in Social Work, Psychology, Human Services, or a related field is preferred.
  • Commitment to non-discriminatory, trauma-informed, and culturally responsive practice; bilingual skills are a plus.
