Division of Administration: Case Management Program
Reporting to: Director of Case Management
Location: Primary work base is the employee’s home office. Secondary work bases are ICDI’s business office at 303 East Wacker Drive, Suite 2108, Chicago, IL 60601 and on-site locations in the greater Chicago metropolitan region. Travel will be required to visit ICDI worksites and conduct outreach in Chicago and the greater metropolitan region.
Work Schedule: The position offers flexibility in terms of working 40 hours per week, but availability during regular business hours is required. This position is required to attend outreach events.
Language Requirement:Superior fluency in Spanish and English
Job Purpose
The Case Management Program team consists of case managers/public benefits specialists responsible for ICDI participant care and support. The Case Manager is responsible for providing case management services, working with short and long-term ICDI clients to identify and address service needs, including housing, benefits, health, and legal, through referral or direct support. ICDI believes in providing compassionate, caring and client-centered services that are culturally competent and linguistically appropriate to all ICDI program clients.
Job Description
Essential Functions & Major Responsibilities:
- Schedule and conduct client intake appointments to help clients identify and prioritize their needs including needs assessments, service planning, goal setting, and expectation setting.
- Refer program participants for mental health services, human trafficking screening, and legal screenings.
- Support in-person and virtual presentations and workshops (cultural orientation, Know Your Rights, etc.).
- Maintain and update case notes, track participant progress, and document site visits and referrals using client management systems (i.e., Salesforce, Google Sheets, Bonterra).
- Make recommendations to the Director of Case Management for mutual aid assistance on an as-needed basis.
- Ensure regular and timely virtual and in-person follow-up with program participants.
- Departure planning and reintegration support for participants who have received an order of removal by an immigration judge.
- Provide translations and interpretations, as needed.
- Complete participant intake questionnaire within 24-48 hours of program arrival to ensure basic needs are met.
- Enroll ad eligible program participants in public benefit programs, including IDHS, and conduct regular follow-up.
- Act as an approved representative before Illinois Department of Human Services (IDHS) personnel, as needed.
- Identify additional city, state, or NGO resources and benefits for participants.
- Assist program participants in scheduling medical and mental healthcare appointments.
- Promote ICDI community outreach through outreach and advocacy events, flyer distribution, and social media.
- Participate in training on benefits eligibility and other trainings, as required by the Director of Case Management.
- Perform other tasks, projects, and assignments which fall within the scope of Case Management as requested by the Director of Case Management, Director of ICDI Programs, and Executive Director.
- Stay current on case management best practices and provide suggestions to the Director of Case Management.
Strategic Responsibilities:
- Provide input, feedback and ideas on the strategy and program implementation of the Case Management team.
- Work with the Director of ICDI Programs to generate data, documentation and reports, as needed.
- Represent ICDI at public speaking engagements.
ICDI Team Responsibilities:
- Contribute to and promote a healthy, respectful, collaborative, adaptable, and flexible organizational culture.
- Participate in symposiums, presentations, trainings, galas, etc.
- Share ideas, learnings, and knowledge to boost creativity and learning amidst the ICDI team.
- Share and communicate newsworthy information and success stories to support the marketing and public relations efforts on behalf of ICDI.
Qualifications
Required:
- Professional/Native bilingual fluency in Spanish and English.
- Valid Driver’s License and Vehicle
- Previous knowledge of state-funded public benefits and how they operate, including Victims of Trafficking, Torturing, and Other Serious Crimes (VTTC) IDHS benefits.
- Familiarity with Google Workspace.
- Experience with case management software, such as Salesforce or Bonterra/Apricot 360.
- 2 to 3 years of experience working in case management with vulnerable populations (including immigrants, refugees, and asylum seekers) in a client/participant service-focused environment.
- Understanding of current social issues within ICDI's client communities and demographic (e.g. domestic abuse, homelessness, anti-immigrant sentiment, etc.)
- Excellent prioritization, critical thinking, problem-solving, communication, interpersonal skills, particularly in challenging situations.
- Demonstrate cultural competence and maintain positive client relationships by recognizing, respecting, and valuing diverse backgrounds, identities, and perspectives of all participants, including but not limited to ethnicity, gender, age, race, religion, disability, sexual orientation, gender identity, language, and culture.
- Experience working in a dynamic, adaptable, and flexible work environment with a team, clients/participants, grant subrecipients, and external partners.
- Strong time-management and collaboration skills.
- Ability to implement wellbeing strategies in a professional environment.
Preferred:
- An Associate or Bachelor's degree is preferred, but not required.
- Experience in identifying, communicating, and addressing program risks and issues.
