SUMMARY:. The Lead Case Manager will organize, monitor, assist and ensure that all delivered services by the Case Management team meet the needs

Duties And Responsibilities

  • Works collaboratively with families and individuals experiencing homelessness to help them establish housing goals and obtain independent housing.
  • Develops SMART Goals with individuals/families and updates progress via Service Plans on a monthly basis
  • Provides linkages and referrals to individuals and families as appropriate to support goals identified in Service Plans
  • Provides clients with appropriate housing opportunities.
  • Assists clients with navigating the administrative bureaucracy surrounding housing, such as obtaining necessary paperwork and completing required applications and forms.
  • Establishes and maintains relationships with landlords and property management companies to facilitate better identification of available housing opportunities.
  • Ensures required case management documentation and forms are collected and completed correctly and accurately
  • Ensures all data entry is completed accurately and correctly within the expected timelines in all required databases (HMIS, AWARDS)
  • Routinely reviews client charts for program compliance to ensure audit readiness
  • Creates case notes, track, and monitor clients’ progress to provide support and ensure their housing goals are met.
  • Works with property owners and representatives to obtain required documents in support of financial assistance requests.
  • Views, inspects, and evaluates potential housing locations to ensure they are well-maintained and appropriate for client habitation.
  • Program Specific: Travels as required.
  • Obtain/Maintain required certification, such as CADC or NCRS.
  • Ensures all residents are signing daily bed sheets/medications logs as required by Program.
  • Conducts regular toxicology screening
  • Provides services and support using the Harm Reduction and Housing First Model
  • Maintains/Obtains CPI Certification
  • Performs other related duties as assigned by management
  • Monitor and screen for daily eligible referrals, assign and monitor case managers caseload
  • Monitor, review and assist Case Managers with master file review and assessment of data collected to assure all necessary documentation is collected for program services eligibility purposes.
  • Monitor, review and maintain HMIS housing programs data entry and required updates
  • Monitor, review and assist in implementation of program and community resources

Qualifications

  • Bachelor's Degree (BA) from four-year college or university, or one to two years of related experience and/or training, or equivalent combination of education and experience.
  • Certificates, licenses, and registrations required:
    • CADC or NCRS preferred, or obtain within 2 years of hire
    • CPI Certification or obtain within the first year of hire
  • Computer skills required: Spreadsheet Software (Excel); Word Processing Software (Word); Electronic Mail Software (Outlook); and Keyboarding.
  • Other skills required:
    • 3+ years of case management experience with Rapid Rehousing, Housing First, or other federal housing program preferred.
    • Knowledge of general principles and approaches for providing services to individuals and families experiencing housing insecurity, substance addiction, interpersonal trauma, and criminal justice involvement.
    • Knowledge of HUD programs, resources, opportunities, and regulations.
    • Knowledge of local community programs, resources, opportunities, and regulations.
    • Knowledge and experience in property management and/or leasing.
    • Program Specific: Ability to travel to various locations on a weekly basis (mileage reimbursed).
    • Ability to remain calm in crisis, hostile, or disaster situations.
    • Able to work well independently and in group settings.
    • Spanish bilingual ability preferred.
