Job summary

Century-Housing in Long Beach, CA is hiring a full-time Case Manager to support residents experiencing homelessness and those at risk. You will coordinate services, conduct assessments, and help residents pursue housing stability using a recovery-oriented approach.

The role requires a bachelor’s degree in human services or related field (or comparable professional experience) and strong communication, organizational, and data-entry skills.

Qualifications

  • Experience working with clients with mental illness, chronic health issues, and substance use disorders.
  • Experience working in permanent supportive housing with low income, diverse populations.
  • Excellent oral and written communication, organization and time management skills.
  • Knowledgeable about eligibility requirements, application procedures and benefits of federal and state entitlement programs.
  • Be able and willing to work flexible hours which may include evenings or weekends.
  • Ability to establish, maintain, track, measure and report program objectives and efficacy to stakeholders.
  • Ability to work with diverse individuals and families with mental disabilities and/or substance addictions; promote autonomy and dignity.
  • Proficient in word processing and database use.

Responsibilities

  • Coordinate and implement support services for a caseload of individuals residing in permanent supportive housing.
  • Develop trusting relationships with residents to facilitate housing placement and stability.
  • Assist in screening, assessment, and application of residents to housing policies and resources.
  • Create individual service plans and conduct comprehensive assessments updated quarterly.
  • Learn and use the CHAMP/HMIS/APRICOT database and maintain accurate progress notes.
  • Maintain knowledge of community resources to provide comprehensive services.
  • Provide interventions and crisis management services, including after-hours crisis response.
  • Provide referrals to primary medical care, mental health services, and other community services.
  • Collaborate with multidisciplinary team and attend weekly meetings.
  • Collaborate with property management to enforce lease requirements and maintain housing.
  • Assist residents in daily activities and appointments to promote recovery and social integration.
  • Obtain training in Housing First strategies and Harm Reduction.
  • Participate in program compliance, evaluation and modification.
  • Promote CVC’s mission, vision, and values; adhere to policies.
  • Provide occasional evening/weekend coverage.
  • Other duties as assigned by supervisor.

GENERAL SUMMARY:Case Management Services form the core of the services for people who are homeless, at risk of homelessness, formerly homeless and who have complex health and/or behavioral health conditions; are high utilizers of public services and other vulnerable populations including individuals with criminal justice histories and individuals who are exiting institutions such as hospitals, residential treatment programs, and custodyfacilities.

The team employs a “whatever it takes approach” to assist a client in their transition from homelessness to housing stability. Case management includes, but is not limited to the following: outreach and engagement, intake and assessment, service planning, linkage to health, substance abuse disorder, mental health and other supportive services. Assistance in benefit establishment, transportation, legal issues and crisis intervention.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
  • Coordinate and implement support services for a caseload of individuals residing in permanent supportivehousing.
  • Develop effective, trusting relationships with residents-with a focus on facilitating housing placement and stability, independence and maintenance of improved physical and mental health.
  • Assist in the screening, assessment, and application of residents including orientation to housing policies, resources, andgoals.
  • Work with clients to create an individual service plan, and conduct comprehensive assessments to be reviewed and updatedquarterly.
  • Learn and utilize the project’s database software (CHAMP/HMIS/APRICOT), maintainup to date, accurate, and complete data and progress notes as required by the project and its fundingsources.
  • Maintain a current, thorough knowledge of community resources and utilize these to provide comprehensive services toresidents.
  • Provide interventions and crisis management services, including after-hours crisisresponse.
  • Provide individualized referrals to primary medical care, mental health services, and other community services asneeded.
  • Work cooperatively and cohesively with other members of the multidisciplinary team, including participation in weekly property meetings, services staff meetings and staff training.
  • Work cooperatively with property management staff to fairly and effectively enforce lease requirements with the goal of maintaininghousing.
  • Assist residents in the management of daily activities and scheduled appointments- facilitating socialization, health maintenance, sense of community, and progress inrecovery.
  • Obtain training in and practice Housing First strategies and Harm Reduction- to help residents be successful in permanenthousing.
  • Participate with administrative staff, in program compliance, evaluation andmodification.
  • Understand and promote CVC’s mission, vision, and values. Adheres to all Century Policies andProcedures.
  • Provide coverage for programs which occur occasionally during evening and weekendhours
  • Other duties as assigned bysupervisor.

EDUCATION AND EXPERIENCE:

Bachelor’s degree in human services, social work, OR comparable professional experience.

Salary and Benefits:

Attractive nonexempt compensation $24.00-$26.00 per hour with fully- paid medical, dental, vision and life/AD&D, long-term care, retirement plan with company match, paid time off, paid holidays and more.
