Job summary

Indiana Professional Management Group, Inc (IPMG) in Huntington County, IN is hiring a full-time Case Manager for a hybrid role with home-office work and frequent local travel to community-based meetings. Applicants must reside in Huntington County and be able to travel across the county and surrounding areas.

You will educate individuals about Medicaid waivers, develop individualized support plans, connect clients with needed services, and lead team meetings with providers to review and adjust

Qualifications

• Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field
• Bachelor's degree in an unrelated field and a minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population
• Valid driver's license, car insurance, and reliable transportation
• Ability to pass thorough background screens
• Designated home office setting with computer, internet service, and smart phone

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve
• Help Individuals access services and supports they need to meet their goals
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed

Education

A Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field

A Bachelor's degree in an unrelated field and a minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population

Let's talk about the Case Manager position!

This full-time, hybrid position combines remote work from your home office with travel of up to approximately one hour for community-based meetings several times a week. This position serves Huntington County, and to accommodate required in-person meetings, applicants must reside within Huntington County, Indiana. Candidates must be able to travel throughout the county and surrounding counties as needed to attend community-based meetings.

Who You'll Work With:

• Case managers work with Individuals on four Medicaid Waivers; Family Supports Waiver (FSW), Community Integrated Habilitation Waiver (CIH), Health & Wellness Waiver (H&W), and Traumatic Brain Injury Waiver (TBI)
