Job summary

IPMG is hiring a full-time Case Manager to serve Harrison County, Indiana, with a hybrid arrangement combining remote work from your home office and in-person meetings across the county several times a week.

Applicants must reside in Harrison County and be willing to travel throughout the county and surrounding counties for community-based meetings. A Bachelor's in a human services field or equivalent experience is required.

Qualifications

• Bachelor's degree in psychology, sociology, social work, or related human services field.
• Or a Bachelor's in an unrelated field with a minimum of one year of direct experience working with people with intellectual/developmental disabilities, or two years with aging population.
• Must have a valid driver's license, car insurance, and reliable transportation.
• Ability to pass background screens and work in a designated home office setting with internet access.

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process.
• Create an Individual support plan based on strengths, needs, goals, and desires of the Individuals served.
• Help Individuals access services and supports needed to meet goals.
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers.

Let's talk about the Case Manager position!

This full-time, hybrid position combines remote work from your home office with travel of up to approximately one hour for community-based meetings several times a week. This position serves Harrison County, and to accommodate required in-person meetings, applicants must reside within Harrison County, Indiana. Candidates must be able to travel throughout the county and surrounding counties as needed to attend community-based meetings.

What You'll Do:

• Provide information and education to help Individuals understand the Medicaid waiver process

Who You'll Work With:

• Case managers work with Individuals on four Medicaid Waivers; Family Supports Waiver (FSW), Community Integrated Habilitation Waiver (CIH), Health & Wellness Waiver (H&W), and Traumatic Brain Injury Waiver (TBI)

Requirements

• A Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field, or
• A Bachelor's degree in an unrelated field and a minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population
• A valid driver's license, car insurance, and reliable transportation
• The ability to pass thorough background screens
• Designated home office setting with computer, internet service, and smart phone
