Job summary

Shelter Care, Inc. is seeking a compassionate Case Manager to support young adults ages 18–21 who have transitioned from foster care.

This role helps clients build independence, develop life skills, and pursue long-term stability and success. Key duties include direct case management, assisting with independent living skills and housing search, crisis intervention when needed, and empowering personal growth.

Qualifications

• Must be at least 21 years of age.
• Must possess a valid driver’s license.
• Bachelor’s degree in social work, human services, behavioral health, or related field, or equivalent experience.
• Must meet and maintain all criminal background check requirements.

Responsibilities

• Provide direct case management and individualized support services.
• Assist young adults in developing independent living and essential life skills.
• Help individuals identify and secure safe, stable, and affordable housing.
• Provide crisis intervention and support as needed.
• Empower young adults to build confidence, strengthen self-sufficiency, and achieve their personal goals.

Education

Key Responsibilities:

• Provide direct case management and individualized support services.
• Assist young adults in developing independent living and essential life skills.
• Help individuals identify and secure safe, stable, and affordable housing.
• Provide crisis intervention and support as needed.
• Empower young adults to build confidence, strengthen self-sufficiency, and achieve their personal goals.

Schedule:

Monday–Friday, with some flexibility required to accommodate the needs of the individuals served.
