Job summary

Memorial Hermann is seeking a Case Management Assistant to support the Case Management team, physicians, and medical office staff. You will help resolve issues, gather information, and participate in outreach to patients and families to promote successful care transitions.

Qualifications include an Associate’s degree (preferred) and 1 year of healthcare experience; licensure options include CNA, LVN, CMA, EMT, or Paramedic. Strong communication and multi-tasking are essential for this role.

Qualifications

• Associate’s degree preferred.
• CNA, LVN, RT, CMA, EMT, or Paramedic licenses preferred.
• At least 1 year experience in a health care setting.

Responsibilities

• Participates in development of patient educational programs with MHMD/CPC committees.
• Provides information and support to Case Management for individualized care plans.
• Acts as liaison between Case Managers and Medical management team for program issues.
• Maintains knowledge of third-party payor standards and NCQA Medical Home Standards.
• Tracks denial types, payor, appeal outcomes and denial trends.
• Supports discharge planning and safe patient transitions from hospital to home.
