Job summary

Memorial Hermann Health System is seeking a Case Manager to support physicians, primary medical homes, and interdisciplinary teams. You will facilitate patient care across the continuum, coordinate discharge planning, and drive performance improvements while managing costs and ensuring high patient satisfaction.

The role requires RN or LMSW licensure in Texas with CM certification within two years. You will work with a multidisciplinary team to optimize outcomes and community care transitions.

Qualifications

• Graduate of an accredited school of professional nursing required; BSN preferred or graduate of an accredited masters of social work program
• Current and valid license to practice as a Registered Nurse in Texas or Current and valid license as a Master Social Worker (LMSW) in Texas; LCSW preferred
• Certification in Case Management required within two years of hire into the Case Manager position

Responsibilities

• Coordinates/facilitates patient care progression throughout the continuum.
• Collaborates with physicians, nursing and other members of the multi-disciplinary care team to effect timely, appropriate patient care.
• Addresses/resolves system problems impeding diagnostic or treatment progress.
• Proactively identifies and resolves delays and obstacles to discharge.
• Collaborates with medical staff and care team to facilitate care for designated caseload and ensures documentation supports the plan of care.
