Job summary

Pathways Inc. in the Gateway region of Kentucky is seeking a Service Coordinator to ensure appropriate services for children and adults with or at risk for intellectual and developmental disabilities.

This full-time, hybrid role requires a Bachelor’s degree in Human Services and involves coordinating a person-centered plan, navigating referrals, and facilitating home visits. Travel within the FIVCO region is required with mileage reimbursement provided.

Qualifications

• Experience working with the IDD population is required.
• Strong documentation and computer skills are required.

Responsibilities

• Determine the nature of referrals and assess the need for additional contacts or screenings.
• Provide intake into the IDDS system.
• Conduct home visits to identify family/individual priorities, resources, and concerns.
• Facilitate, develop, review, and monitor person-centered plans on an ongoing basis.
• Coordinate, monitor, and ensure timely delivery of services outlined in the person-centered plan.
• Maintain thorough documentation and comply with all agency policies.
• Perform other duties as assigned in the current job description.

Salary: $35,000

Educational Requirements:

• Minimum of a Bachelor’s degree ina Human Services field.
