Greater Reach Community Services Board in Petersburg, VA, is seeking a full-time Support Coordinator - ID to connect individuals with intellectual disabilities to needed supports and services for independent living and community thriving. Responsibilities include case management, service coordination, and conducting home visits and assessments for both children and adults with ID/DD.

Minimum HS diploma; driver’s license required. A Bachelor's in human services is preferred.

Competenze

• Minimum High School Diploma or GED required.
• 1 year of experience working with individuals with ID/DD preferred.
• Valid Driver's License required, with a point balance of -6 or higher.

We are recruiting for a full-time Support Coordinator - ID. This position plays a vital role in improving the lives of individuals with intellectual disabilities by connecting them to the supports they need to live independently and thrive in the community.

Responsibilities

Provide case management and service coordination for children and adults with intellectual disabilities, assist individuals in accessing medical, psychiatric, social, educational, vocational, and residential services, and conducting home visits and comprehensive assessments.

Requirements

• One ( 1) year experience working with individuals with ID/DD diagnosis.
• Valid Drivers License with a point balance of -6 or higher.

Preferred

• Bachelor's in human services .

Fingerprints, State and FBI criminal record reports, drug test, and central registry report will be required upon selection.Our Agency maintains a drug-free workplace.

Annual Salary Range

Grade S-C7

$52,576.00 - $59,148.00

$3,000 Sign on Bonus Restrictions Apply
