Pathways Inc. - Kentucky is seeking a Service Coordinator for a hybrid position in the Gateway region to help children and adults with, or at risk for, intellectual and developmental disabilities.

The role focuses on coordinating services and ensuring person-centered planning. Minimum qualifications include a Bachelor's degree in a Human Services field, strong knowledge of community resources, and experience with IDD populations.

• Provide intake into the IDDS system.
• Conduct home visits to identify family/individual priorities, resources, and concerns.
• Facilitate, develop, review, and monitor person-centered plans on an ongoing basis.
• Coordinate, monitor, and ensure timely delivery of services outlined in the person-centered plan.
• Maintain thorough documentation and comply with all agency policies.

Position Overview
