Qualificações

• Associate Degree in Nursing (ADN) is required.
• Current Massachusetts Registered Nurse (RN) license is required.
• Minimum of three years of case management experience.

Responsabilidades

• Oversee daily case management and utilization review workflows, assignments, and staffing.
• Lead orientation, training, scheduling, performance management, and professional development.
• Provide guidance on evidence-based practices, clinical guidelines, and case management interventions.
• Participate in case management and utilization review committees and communicate issues to Physician Advisor.
• Coordinate with Denials and Appeals Specialists to resolve utilization management concerns.
• Assess admission appropriateness, level of care, and continued stay using screening criteria and guidelines.
• Communicate findings to the healthcare team and involve Physician Advisor when needed.
• Monitor patient progression, length of stay, and discharge planning to support transitions.
