Ascension Seton Hays in Kyle, TX is seeking a dedicated Social Worker to join the Case Management team. This full-time, days role leads the design of personalized care plans from admission to discharge, ensuring alignment with patient values and recovery goals.

You will advocate for patients by removing barriers to care, support families through crises, and collaborate with an interdisciplinary team to document care and improve outcomes.

Formación

• Bachelor's degree in Social Work required.
• Social Worker credentialed from the Texas State Board of Social Worker Examiners obtained prior to hire date or job transfer date.
• CCMC preferred; ACM accepted.

Responsabilidades

• Lead the design and execution of personalized care plans from admission to discharge reflecting patient values and recovery goals.
• Advocate for patients by identifying and removing social or financial barriers to care.
• Support families through complex psychosocial challenges and crises, providing expert guidance.
• Collaborate with the interdisciplinary care team to refine treatment strategies and maintain high-quality medical documentation.
