Moments Hospice in Davenport, IA is seeking an RN Case Manager to lead clinical care for patients and families. You will manage symptoms, coordinate interdisciplinary care, and guide families through the final chapter of life with dignity and comfort.

The role offers a full-time schedule, a Monday–Friday work week, and a fleet car program for business and personal use. Prior hospice experience is advantageous, and leadership opportunities are available.

Qualifications

• Active RN license and eligibility to practice in Iowa.
• Experience in hospice or palliative care preferred.
• Ability to coordinate patient care across disciplines.
