Job summary

Southwest General in Middleburg Heights, OH, seeks a Case Management Manager to lead the daily operations of the Case Management Department, including staffing, workflow oversight, discharge planning, and performance outcomes. Qualifications

BSN required or equivalent per policy.
Master's degree in Nursing, Healthcare Administration, Business Administration, Public Health, or related field preferred.
Five+ years of progressive case management/discharge planning experience in acute care is preferred, with formal leadership experience required.

Responsibilities

Provide daily operational leadership of the Case Management Department, including staffing, workflow oversight, patient progression, and discharge planning.
Ensure regulatory compliance and department performance outcomes; coach and supervise CM staff; collaborate with physicians, payers, and leadership to support evidence-informed decisions.
