Job Description

The RN Care Manager can contribute to the assessment of health status through gathering and recording of assessment data, observing, monitoring, and reporting changes in condition. Conduct interviews to understand cause and effect, gather or review health history for clinical symptoms, and determine health needs. The RN Care Manager collaborates with internal and external stakeholders on managing a successful transition from hospital to home. The RN Care Manager is responsible for completing timely post-discharge patient outreaches to identify, develop, implement, and assist with interventions for high-risk patients. The RN Care Manager supports and manages the post-discharge needs to reduce hospital readmission, improve patient care engagement, and enhance patient experience. This is a remote position with standard hours.

Qualifications:

• 2+ years of experience as an RN case manager
• Active California RN License (LVN/LPN license not accepted)
