About The Position

During visits lasting up to one hour, providers review health, medical history, medication adherence, social environment, and other risks, providing insights that improve care quality and help close care gaps.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment, education, referrals, and documentation
• Collaborate with healthcare team members and family caregivers
• Coordinate with Primary Care Physician on patient education and follow-up
• Provide services in various settings such as home visits and skilled facility visits

Position Details

Type: PRN - Flexible, Nurse Practitioner

Pay: Earn from $2,200 up to $11,000/month based on assessment volume and pay rate of $110 per assessment

Location: Home, SNF, community environments in Waldorf, MD

Hours: Days, Evenings, Nights, Weekends, Flexible Scheduling

Benefits: Non-benefited except for 401(k) and voluntary benefits; mileage reimbursement included

Qualifications

• Master’s Degree or equivalent experience with NP licensure
• Current RN and NP licensure in the state of practice, with prescriptive authority
• Board certification by AANP, ANCC, or AACCN in an approved specialty
• Current BLS, ACLS, or CPR certification
• 1 year NP experience preferred; new grads encouraged to apply
• Strong computer skills and familiarity with medical record software
• Excellent communication skills
• Flexibility with change

Travel Requirements

• Travel to patient homes required
• Valid driver’s license, vehicle insurance, and ability to travel
