Job summary

The ideal candidate will have significant experience in acute care settings and will oversee staff while ensuring effective patient care coordination and resource utilization.

This full-time position offers a competitive salary of up to $105,000 annually, relocation assistance, and comprehensive benefits. Candidates must have a Bachelor’s in Nursing or a Master’s in Social Work and a strong background in hospital-based case management.

Qualifications

• Active, unrestricted RN or LCSW license in the state of practice.
• Minimum 3 years of acute care clinical experience.
• Minimum 2 years of supervisory or leadership experience in an acute care setting.
• Strong knowledge of hospital-based case management.

Responsibilities

• Provide leadership and oversight of the Case Management program.
• Supervise and mentor RN and social work staff.
