Job summary

Select Medical's Critical Illness Recovery Hospital in Savannah, GA is seeking a Director of Case Management to lead utilization review, discharge planning, and care coordination for medically complex post-ICU patients.

Qualified candidates will hold an active clinical license (RN/RT) or a master’s in social work, have 3+ years in medical case management with supervisory experience, and demonstrate leadership in a hospital setting. Full-time position with competitive benefits.

Qualifications

• Active clinical license (RN/RT) or MSW required.
• Minimum 3 years in medical case management or similar.
• Prior leadership experience in a hospital setting preferred.

Responsibilities

• Lead Utilization Review and resource management to optimize care and cost.
• Develop and manage patient-specific discharge plans.
• Coordinate with payors to ensure proper reimbursement and access to services.
• Ensure compliance with Utilization Management policies and regulatory requirements.
• Oversee UR compliance with Quality Management and licensure/accreditation standards.
• Facilitate multi-disciplinary team meetings with physicians, nurses, and therapists.
