Job summary

Mountain View Hospital in Idaho Falls seeks a Discharge Planner to coordinate discharge plans and services for patients, families, and care teams, ensuring a safe, effective transition from acute care.

You will assess patients' needs, arrange community resources, advocate for patients, maintain records, and participate in case management while aiming for cost-efficient care.

Qualifications

• Graduate from an accredited program with an active Idaho LPN/RN license or a BA in Social Work.
• Experience in discharge planning or case management preferred.
• Excellent communication with patients, families, and care teams.

Responsibilities

• Coordinate discharge plans with patient, families and disciplinary teams.
• Review and validate orders, report progress to supervisor.
• Assist with referrals to community resources and post-hospital needs.
• Maintain patient records and ensure confidentiality.
• Contribute to case management activities during and after hospitalization.
• Support supervision of resource utilization for quality, cost-effective care.

Mountain View Hospital is looking for a Discharge Planner join our team!JOB SUMMARY:Works in close collaboration with physician health care personnel in patient evaluation and treatment to further their understanding ofsignificant social and emotional factors underlying patients’ health problems. Prepares patient and family for safe discharge fromacute care hospital setting.Duties and Responsibilities1. Coordinate the discharge plan and services with the patient, their families and disciplinary teams to ensure that a patient has an appropriate plan and services when ready to discharge.2. Work with patients while obtaining maximum benefits from medical care by performing the following duties.3. Reviews and validates physician orders are carried out, reports progress and unusual occurrences on patients to department supervisor.4. Ensures cost effective and appropriate health care services are provided to all patients regardless of patients funding.5. Collaborates with other departments in discharge planning.6. Assists in assessing the physical, functional, social, psychological, environmental and financial needs of patients.7. Assists with referrals to appropriate community resources and contributes to the assessment of patient’s support systems.8. Acts as a patient advocate by assisting in acquiring appropriate medical, nursing or other health care in the home or clinic setting as needed upon discharge.9. Maintains accurate patient records, ensuring patient confidentiality.10. Contributes and assists in the performance of case management activities, and contributes to the assessing, planning, implementing, coordinating, monitoring and evaluating of the needs of patients for services both during and after hospitalization.11. Or, a Licensed Social Worker, bachelor’s degree (B.A.) in social service from four-year college or university; preferred experience in a healthcare facility.Experience:3+ years of clinical nursing.Equipment/Technology: Basic computer knowledge.Language/Communication: Effective written and oral skills. Ability to comprehend and follow detailed instructions. The ability to effectively present information and respond to questions from groups of managers, patients, customers, and the pressure during difficult circumstances. Speaks clearly and persuasively in positive or negative situations listens and gets clarification.
