Responsibilities

  • Conduct comprehensive assessments of members’ needs and eligibility using care management tools and data review
  • Recommend case-resolution approaches and evaluate legal, educational, biopsychosocial, benefit-plan, and resource needs
  • Identify high-risk factors and service needs affecting member outcomes and care planning
  • Refer members to clinical case management or crisis intervention when appropriate
  • Coordinate and implement assigned care-plan activities
  • Monitor and update care plans to reflect progress, health, wellness, and compliance with juvenile justice system guidelines
  • Collaborate with state and behavioral health system partners to help members understand their healthcare needs
  • Consult with case managers, supervisors, Medical Directors, and other health programs to overcome barriers
  • Present cases at multidisciplinary case conferences
  • Identify and elevate quality-of-care issues through established channels
  • Negotiate appropriate options and services to meet members’ benefits and healthcare needs
  • Use motivational interviewing to promote member engagement and lifestyle or behavior changes
  • Coach, inform, and support members in making independent medical and healthy-lifestyle choices
  • Help members participate knowledgeably in healthcare decision-making with providers
  • Apply case management and quality management processes in compliance with regulatory, accreditation, and company guidelines

Job description

Requirements
  • Qualified candidates must reside in Breckinridge, Grayson, Meade, Hardin, Larue, Bullitt, Nelson, Marion, Washington, Spencer, Anderson, Woodford, Shelby, Franklin, Henry, Oldham, or Trimble counties in Kentucky
  • 2 years’ experience in behavioral health, social services, or human services field
  • 1+ year(s) of experience with Foster Care and juvenile justice systems, Adoption Assistance, delivery of Behavioral Health Services, Trauma‑informed Care, ACEs, Crisis Intervention services, and evidence-based practices applicable to Kentucky SKY populations
  • 1 year experience with MS Office Suite applications, including Word and Excel
  • Valid KY driver's license
  • Proof of vehicle insurance
  • Reliable transportation and personal vehicle
  • Willingness and ability to travel 80-90% of the time for face-to-face member visits
  • Flexibility to work beyond Monday-Friday, 8am-5pm core business hours
  • Minimum of a bachelor's degree or a non-licensed master level clinician degree in behavioral health or a relevant human services field (social work, psychology, marriage and family therapy, or counseling)
  • Direct/hardwired internet connection within 7 feet of the modem/router, with minimum speeds of 25 Mbps download and 3 Mbps upload; WiFi and satellite internet are not permitted
  • Quiet, secure, private designated home virtual work location compliant with CVS Health and HIPAA guidelines
  • Ability to work within the state and city where currently residing
  • Ability to use multiple monitors and navigate multiple applications simultaneously
  • Ability to communicate via email, calendar invites, Teams messaging, and virtual meetings
  • Experience with Microsoft Office 365 applications or similar Google Workspace tools
  • Ability to log in to secure systems such as VPN and EHR portals, maintain computer security, manage strong passwords, and recognize suspicious emails or links
  • Ability to troubleshoot common technical issues independently
