CareAllies, Inc. is seeking a remote-enabled liaison to perform telephonic outreach for recently discharged and high-risk patients, coordinate discharge plans, and identify treatment options that support quality, cost-effective care.

You will consult with physicians, providers, and members to assess needs, coordinate services, and monitor progress. The role may travel within the region and can be performed remotely from the continental United States, excluding CA, NY, AK, and HI.

Facilitating discharge coordination and identifying alternate treatment programs that provide medically necessary potentially cost-effective services. Consulting with physicians, providers, members, and other resources, as appropriate, to assess, plan, facilitate implementation, coordinate, monitor, and evaluate options and services required to meet an individual’s health needs, using communication and available resources to promote quality, cost effective outcomes. Serving as liaison to physicians, and members. This position provides telephonic support for all locations.
