Encompass Health Corporation in Columbus, GA seeks a Case Management Assistant to streamline workflow, improve communications, and support the Case Management department across inpatient rehabilitation settings.

You will engage with local community resources, coordinate team meetings, and help with discharge activities to ensure coordinated care for patients and families. This role supports growth and commitment to patient-centered care.

Qualifications

• Proficiency in Microsoft Office products.
• Strong typing skills with speed and accuracy.
• Knowledge of medical terminology (preferred).
• Experience with insurance/payor systems (preferred).

Responsibilities

• Coordinate and facilitate Team Conference meetings.
• Gather, take action on, and maintain Case Management documentation such as referrals, admissions, orders, and schedules.
• Effectively coordinate communication of daily Case Management Assignment updates.
• Complete pre- and post-discharge activities to ensure patients, families, and clinicians are aware and prepared.
• Gather Quality Improvement and department Performance Improvement data.
• Assist with collection of Key Care Indicators and clinical outcomes data.
• Assist in regulatory and joint commission survey preparedness activities.
