Job summary

Trinity Health is seeking a Care Coordination Support role in Fresno, CA to assist with utilization review management and discharge planning. You will help screen for medical necessity and coordinate with the Care Coordinator for non-complex discharges.

Requirements include a high school diploma, 2 years in acute care or clinical settings, strong interpersonal skills, medical terminology competency, and CPR certification. Prior case management experience is preferred.

Qualifications

• High school diploma or equivalent is required.
• Two (2) years of experience in an acute care hospital or other clinical care setting is required.
• Well-developed interpersonal skills with the ability to communicate effectively/professionally with staff, patients, family, visitors, and physicians under routine and stressful situations are required.
• Medical terminology course completion or demonstrated functional competency is required.
• Computer data entry experience is required.
• Current AHA Healthcare Provider CPR card is required.
• Experience with case management, utilization review, and/or discharge planning processes is preferred.

4. Successful completion of a medical terminology course or demonstrated functional competency is required.
5. Computer data entry experience is required.
6. Current American Heart Association (AHA) Healthcare Provider CPR card is required.
7. Experience with case management, utilization review, and/or discharge planning processes is preferred.

Pay Range: $23.00 - $29.61
