Job description

Overview

Performs all activities related to support of the Case Management Department. Responsible for performing clerical, communications, and follow‑up activities for and with staff, individuals/families served, and providers as assigned in a confidential and timely manner.

Responsibilities
  • Provide clerical & administrative support, including mailing, faxing, and scanning items as needed for the agency.
  • Research resources and complete referral forms.
  • Facilitate communication with individuals/families served via phone and in person.
  • Assist with Medicaid applications and prepare electronic forms as needed.
  • Process required documents according to program guidelines.
  • Develop and maintain accurate knowledge of program requirements, HIPAA and agency confidentiality policies, and relevant tools, software, and hardware platforms.
  • Serve as part of front‑desk rotation, greeting visitors and routing them to appropriate staff during normal business hours.
  • Demonstrate competency by meeting or exceeding department work performance requirements.
  • Communicate with supervisor daily to report concerns, incidents, or issues.
  • Serve as an example of agency standards of excellence, exemplifying vision and goals.
  • Complete and attest to the accuracy of payroll records and submit for review per agency schedule.
  • Maintain professionalism in relationships with customers, co‑workers, and supervisors.
Education & Experience

Required: Associate’s degree from a two‑year college or technical school and two years of experience and/or additional education in human services or a related field; or a high‑school diploma or GED plus four years of related experience and/or training in healthcare and/or customer service with advanced technology tools.

Preferred: Experience working with older adults, in healthcare, or in a Medicaid/Medicare setting; familiarity with state agencies and Medicaid approval processes; demonstrated background in administrative support.

Eligibility & Requirements
  • Possess a valid driver’s license and current automobile insurance, as specified by the agency.
  • Provide an annual negative TB certificate or documentation of exemption.
  • Comply with vaccination requirements (e.g., flu) if duties require service in hospitals or long‑term care facilities.
Work Hours

This is a full‑time non‑exempt position. Agency business hours are Monday through Friday, 8:00 am–4:30 pm.

Travel

Travel throughout the agency’s service area during business hours.

Physical Demands

The individual must be able to communicate effectively, sit or stand for extended periods, and transport and use a laptop and client files.

Work Environment

Operates in a professional office environment with standard office equipment and also visits client residences.
