Job summary

You will conduct screenings, build person-centered plans, coordinate care, and present information clearly to the treatment team.

Responsibilities include counseling, discharge planning, documentation and collaboration with referral resources, with a focus on safety, confidentiality and quality of care.

Qualifications

• Provide assessments and planning with client-centered approach.
• Complete required documentation at time of service.
• Coordinate care and discharge planning.

Responsibilities

• Provide screenings, interviews and assessments in a professional manner.
• Develop Person Centered Planning and treatment planning for each client.
• Establish therapeutic counseling relationships and facilitate counseling.
• Coordinate with referral resources and discuss discharge plans.
• Document progress notes within one working day and maintain chart integrity.
• Attend division team meetings and share relevant client care information.
• Participate in urine collection as requested and use results therapeutically.
