Job summary

Net-Centers-Net-Community-Care is seeking a Counselor to provide assessment and treatment services in Philadelphia, maintaining an active caseload and clinical charts per PA DOH and CARF standards. The role emphasizes accurate documentation, compliance, and coordinated care within contracted guidelines.

Responsibilities include LOC determination, ongoing client sessions, and comprehensive recovery planning, with routine drug screening and adherence to regulatory requirements.

Qualifications

• Annual training totaling 25 hours in areas supportive of NET and PA DOH skill development.
• Ten hours of training on HIV, STDs and TB.
• If licensed/certified, maintain licensure and certification; CCDP recommended.

Responsibilities

• Provide assessment services using instruments to determine level of care (LOC).
• Maintain an active census and schedule clients on a weekly basis when appropriate.
• Develop Comprehensive Recovery Plans with Progress Notes and Reviews.
• Ensure documentation aligns with DOH Licensure standards and CARF requirements.
• Manage admission into LOC and discharge documentation accordingly.
• Perform Urine Drug Screens and other duties as assigned.
