We’re looking for a highly organized professional to be our “go to” for the daily unit operations of discharge planning and case management.
Your main responsibility will be to the people we serve, ensuring that policies and procedures are followed, and that benchmarks for performance are met in an efficient and effective manner. As such, you’ll have a strong relationship building with both internal and external providers and resources.
Our Team Lead provides recovery-oriented support to individuals with Serious Mental Illness transitioning from inpatient psychiatric hospitalization to our program before reintegrating back to community living situations. We’re looking for people who have experience developing and mentoring a team, who believe in a strengths-based approach, and use positive practices to help guide their everyday activities. The Extended Acute Care is a licensed, 16-bed temporary residential treatment program.
Monday-Friday 8am-4pm
